How to Schedule an Appointment with Us

Managing an appointment with us is straightforward. If you need to schedule, reschedule, or cancel an appointment, please contact your preferred location directly. 

If you can’t keep your appointment, please provide us with a 24-hour notice to avoid a $50 no-show fee, which is not covered by insurance.

Insurance and Billing

When you come to us for chronic and/or acute pain management, we don’t want you to feel concerned about how you’ll pay for the care you need. That’s why we strive to simplify your financial interactions. Continue reading to learn more, or download our financial policy for your convenience.

Payment Methods

We accept most payment methods, including cash, check, money order, credit cards, and debit cards. For those without insurance, a down payment is required before your appointment. If you face difficulties paying your bill, please contact our Billing Office to discuss payment plans.

Understanding Your Financial Responsibilities

Insurance Verification: While we verify that you have an active insurance policy, this doesn’t guarantee payment. It’s important for you to understand your insurance coverage and benefits. We recommend contacting your insurance company to confirm your plan details and coverage for pain management treatment.

Billing Process: We’ll bill your insurance company directly on your behalf. However, you’re responsible for any co-pays, deductibles, and non-covered services. Be sure to bring your insurance card to each visit.

Patient Responsibilities: You are required to provide us with updated personal information, such as your current address and phone number. If your visit is related to an injury, provide details like the date of injury and workers’ compensation information. Pay your bill within 20 days of receiving a statement to avoid additional charges.

Non-Covered Services: Unfortunately, insurance companies don’t cover all medical services. For any services not covered by your insurance policy, you are responsible for payment. We can help determine if a service is covered by your plan prior to your appointment.
